We use it only to call you back about your work injury. We do not sell it, rent it, or share it with advertisers. Please do not include medical details in the form; save those for your visit.

Your health information (HIPAA)

Once you become a patient, your medical records are protected health information under the Health Insurance Portability and Accountability Act (HIPAA). We use and disclose it only for treatment, payment and health care operations, and as required to process your claim with the U.S. Department of Labor, Office of Workers' Compensation Programs, which you authorize when you file a federal work injury claim. Any other disclosure requires your written authorization, which you may revoke at any time.

You have the right to see and get a copy of your records, to ask us to correct them, to ask for restrictions on how we use them, to ask us to communicate with you in a specific way, and to get a list of certain disclosures. Our full Notice of Privacy Practices is available at the front desk and on request.
